A WPATH letter is a clinical assessment letter from a qualified mental health professional confirming that a meets the criteria for gender-affirming surgery as set out in the World Professional Association for Transgender Health (WPATH) Standards of Care. WPATH letters are used by some surgical providers and many NHS gender pathways as part of the pre-surgical assessment process.

You do NOT need a WPATH letter to access top surgery at Centre for Surgery. We operate on a self-referral model — adults aged 18+ can book a consultation directly without any letter, referral, or formal diagnosis. Do you need a WPATH letter for top surgery in London?

It depends on which provider you use. WPATH letter requirements differ significantly between the NHS gender pathway, private providers that follow WPATH SOC strictly, and private providers that use a self-referral model.

For all gender-affirming surgery at Centre for Surgery — , , , , body feminisation, body masculinisation, and non-binary surgery — adults aged 18+ self-refer directly. No WPATH letter, GP referral, gender clinic letter, or formal diagnosis of gender dysphoria is required.

A mandatory cooling-off period applies after every consultation before surgery is booked.

NHS surgery is accessed via the specialist Gender Identity Clinics (GICs). Patients on the NHS pathway are over multiple appointments and receive WPATH-style assessment letters as part of the formal pre-surgical process. NHS waits are typically 12–18 months or longer for an initial GIC appointment, with time before surgery is .

What is the WPATH Standards of Care?

The World Professional Association for Transgender Health (WPATH) is an international, multidisciplinary professional association that publishes the of Care (SOC) for the health of transgender and gender-diverse people. The assessor should be familiar with WPATH SOC v8 specifically.

The assessment is typically a single appointment of 60–90 minutes, sometimes split across two appointments depending on the assessor. The clinician will discuss your gender identity, history, current circumstances, the procedure you are considering, your understanding of risks and benefits, and your readiness to proceed.

If the assessor you meet WPATH SOC v8 criteria, they will issue the letter — typically within 1–2 weeks of the assessment.

No. WPATH SOC v8 does not require a specific duration of hormone therapy before top surgery, and hormone therapy is not a precondition. Many patients have top surgery without ever taking hormones.

If an assessor concludes you do not currently meet criteria, they should explain why and outline what additional support, time, or steps might be appropriate. You can also seek a second opinion from a qualified assessor.
